You can write down the chords you're using in the boxes on the upper left. And you could use the upper right six line thingies to jot down riffs maybe. (Maybe label them 1,2,3,... or +,- ,Δ... whatever...

The rest is to write your song down, the smaller line segments are for chord or riff identification, the larger are for your lyrics.
